Devoted Health combines Medicare Advantage coverage with a different kind of member experience — built around your health plan, Devoted Guides and Devoted Medical.
But the right plan still comes down to the fundamentals: your doctors, hospital, prescriptions, costs, network and eligibility for additional benefits.

The Devoted experience can be understood through three connected layers.
Determines your network, hospitals, copays, MOOP, prescriptions, dental, vision, hearing, OTC and supplemental benefits.
The plan documents control coverage.
Help members understand benefits, resolve billing questions, find lower-cost Rx options, manage medications, schedule appointments and locate resources.
Guidance does not override plan rules.
Devoted’s in-house clinical organization can provide virtual care and, where available, in-home support alongside existing physicians.
An additional clinical layer.
Devoted members can access Care OnDemand through Devoted Medical at no additional member cost. It is another point of care — not a replacement for emergency services.
Works alongside your regular physician and provides an additional layer of clinical support.
In applicable markets, members can choose Devoted Medical for full virtual primary care. In that situation, Devoted Medical becomes the designated PCP.
Support = your existing doctor remains your regular physician. Primary Care = Devoted Medical becomes your PCP.
| Question | Devoted HMO | Devoted PPO |
|---|---|---|
| Provider network | Yes | Preferred network |
| Routine out-of-network care | Generally limited | Often available at higher cost |
| Specialist referrals | Depends on state/plan | Generally no |
| PCP relationship | Often important | More flexible |
| Medical MOOP | Yes | Yes |
| Drug coverage | Common | Common |
Routine covered care generally comes from participating network providers. For 2026, certain HMO members in Florida, Illinois and Texas may need referrals for certain specialists.
PPO coverage can provide greater potential out-of-network flexibility, but PPO does not mean every doctor must accept Devoted. Out-of-network care can also cost more.
Compare networks, referrals and out-of-network flexibility.

Don’t ask only “Do you take Devoted?” Ask: “Are you participating with this exact Devoted Medicare Advantage plan?”
Check PCP, specialists, hospital, imaging, surgery center, physical therapy, lab and skilled nursing facilities.
Most Devoted Medicare Advantage plans include Part D, but “Includes Part D” does not mean “covers all my medications.”
Check medication, tier, prior authorization, step therapy, quantity limits and pharmacy.
Devoted uses CVS Caremark for certain Part D pharmacy-benefit administration. That does not mean Devoted is owned by CVS.
Current plans include a dental allowance. Verify dentist access, treatment categories, annual allowance, reimbursement rules and exclusions.
Plans can include routine vision exams and allowances for glasses or contacts. There is no universal Devoted eyewear allowance.
Check hearing-aid allowance, approved devices, provider rules, replacement frequency and member copay.
Devoted currently states SilverSneakers membership is available at no cost across its Medicare membership.
Some plans reimburse eligible wellness purchases and services. The annual amount varies by plan.
Many plans include an allowance for approved OTC health products. Exact amounts are plan-specific.
Certain plans can provide an allowance toward approved healthy food, housing-related expenses, utilities and other qualifying categories.
Not every Devoted member qualifies. Food & Home is an eligibility-based Special Supplemental Benefit for the Chronically Ill.
Designed for qualifying people with Medicare + Medicaid. Medicaid categories can include QMB, QMB+, SLMB, SLMB+, FBDE and QI.
Don’t determine eligibility from the word “Medicaid” alone.
Designed for qualifying chronic conditions such as diabetes mellitus, chronic heart failure and certain cardiovascular disorders.
A treating provider must verify the qualifying condition within the applicable timeframe.
Some Devoted plans can reduce part of the member’s Part B premium, but don’t compare the Giveback by itself.
Serious-illness exposure
Inpatient cost
Frequent specialist care
MRI / CT / PET
A larger Giveback or supplemental allowance is not automatically a better Medicare plan.
Devoted operating in multiple states does not mean one plan provides unrestricted nationwide routine care.
Verify doctors and hospitals in both locations, HMO/PPO rules, out-of-network costs, combined MOOP, pharmacy access and prescriptions.
